To obtain a Certificate of Insurance or “Proof” of Insurance, simply send a fax, on letterhead , to 813-222-4288 or an email to psp@bbprograms.com that includes the following:

• Named Insured's policy number
• What Coverage(s) need to be listed (Professional Liability, Property, Wind etc.)
• Where to send the Certificate to (Fax, Email, Mail etc.)
• Signature of the Named Insured
